Community Case Manager RMN/RNLD – Complex Care, London
We are seeking an experienced Community Case Manager RMN/RNLD to join a respected provider of adult and paediatric complex care in London. This is a full-time role, Monday to Friday (9–5), with hybrid working available – two days from home and three days in the community. The position offers a competitive salary of £50,000–£60,000 DOE.
As a Community Case Manager RMN/RNLD, you will oversee community-based complex care packages for both paediatric and adult clients. Working closely with families, clients and commissioners, you’ll ensure care is safe, personalised and of the highest standard.
Key Responsibilities:
Manage and oversee complex care packages across the community.
Mentor and train nurses and HCAs to strengthen skills and service delivery.
Develop and maintain strong relationships with commissioners and ICBs.
Act as a visible and reliable point of contact for clients, families, and staff.
What we’re looking for:
Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD) with active NMC registration.
At least 2 years’ post-qualification experience.
Background in comunity mental health or Tier 2 CAMHS.
Full UK driving licence and access to a vehicle.
