Job summary
We are seeking to appoint a dedicated and motivated Health Visitor to work within Belper and Derby City area.
The Belper 0-19 Team has been awarded Gold accreditation for their work with Families and Communities.

Working in a part rural community, you will find many opportunities for developing public health work with communities and partner agencies, as well as delivering the core contacts within the Healthy Child Programme.
The right candidates will hold the SCPHN qualification in Health Visiting and be innovative and dynamic team players seeking a new challenge.
This post is 30 hours per week.

Main duties of the job
1. To provide Health Visiting services to families in the community team caseload by delivering the Healthy Child Programme.
2. To manage a defined active caseload and take responsibility for safeguarding children issues within that caseload.
3. To assess individual needs and develop, implement, and evaluate care programmes.
4. To manage a team of Health Visiting support staff.
