The North Liverpool Primary Care Network would like to recruit a Wellbeing Link Worker (Social Prescriber) to make a new team. This role will be based over the seventeen member practices of the PCN.
Main duties of the job
Social prescribing empowers people to take control of their health and wellbeing through referral to link workers who give time, focus on what matters to me and take a holistic approach to an individuals health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.The post holder will also support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.The post holder will work as a key part of the primary care network (PCN) multi-disciplinary team. Social prescribing can help PCNs to strengthen community and personal resilience and reduces health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing peoples active involvement with their local communities. It particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

The North Liverpool PCN is based in the North of Liverpool covering 21 GP practices with an approx. list size of approximately 137,00. We work closely together to strive and provide a proactive, accessible and coordinated care within our local community.
Job responsibilities
To support patients of the North Liverpool Primary Care Network (NLPCN) and promote lifestyle changes, preventing ill-health by means of behavioural change techniques. The role will provide information and support to patients in addition to becoming the link between the patient, GP and other service providers. The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals.

Qualifications
* Educated to GCSE level or equivalent
* Experience of working with the general public
* Experience of working in a health and social care setting or with relevant voluntary sector organisations
* Experience of delivering lifestyle changes interventions
* Experience of working with vulnerable people
* Excellent communication skills (written and oral)
* Competent in the use of Office and Outlook
* Effective time management (planning and organising)

* Good understanding of holistic approach to social prescribing and wider determinants of health
