A Support Planner is needed to ensure business continuity and collaborative working with health partners to provide timely and effective support for residents of Thurrock. This role involves carrying out statutory duties under legislation such as the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005, focusing on assessment of care and support needs and safeguarding vulnerable individuals.
The postholder will ensure residents' needs are met promptly to prevent deterioration and hospital admission, work closely with health colleagues for timely hospital discharges, and support carers and families through timely interventions. Collaboration with health and other partners is essential to avoid inappropriate or premature admissions to hospitals or care homes and to facilitate safe discharges.
An Enhanced DBS check with Adults Barred List is required for this position.
