Job summary
Are you passionateabout improving wellbeing?We are looking for an organised, self-motivatedand confident individual to join our Social Prescribing team, helping adult andchild patients to access support that will help them lead more independent,resilient and healthier lives.
Main duties of the job
Working alongside GP practices, SocialPrescribing Link Coordinators help patients and families connectwith health and community services which will make a real difference totheir emotional and physical wellbeing.
You will deliver personalised support topatients, independently managing new referrals from health professionalsand a caseload. It is essential that you have an understanding of how toprioritise need and are able to recognise when to refer on to specialistservices, being confident to communicate at all levels acrosshealth and social care sectors.
Theposition is available in north Kent and applicants must have experience ofworking with adults or children, a background in medical or social care, andexperience of delivering health and wellbeing support to clients or groups.You will need todemonstrate a robust knowledge of confidentiality, data protection,safeguarding and boundaries in all aspects of your work.

Job responsibilities
* Independently manage GP referrals,self-referrals and online enquiries for people with health conditions(including common mental health conditions, obesity, diabetes, respiratoryconditions, mobility issues and sensory impairment)
* Work with children, families and patientsin a supportive, holistic way to address practical and psychological barriers,such as lack of transport, low confidence and social isolation, copingstrategies for daily living; work together to co-produce a solution
* Support communication with young people andadults, their families and between services; advocate for young people andadults at risk
* Make referrals to local community groupsand other agencies; facilitate the sharing of information, resources andintelligence about any gaps in provision
* Accurately represent the PCN and ensure thevalues of the PCN are always upheld in carrying out the role
* Develop the role in line with therequirements of the PCN; undertake activities to meet delivery needs; work withclients on a one-to-one basis; deliver group support where required
* Develop networks and work in partnershipwith other agencies; attend multi-disciplinary meetings
* Signpost and facilitate access to supportservices including financial and benefit advice, housing, health and safetyhome systems, and leisure and social activities
* Work with GPs and health and social careprofessionals to increase awareness of the services; attend local andnetworking events
* Take part in comprehensive induction,training and development, adhere to Imago policies, and work with anunderstanding of lone working, data protection, and health and safetyprinciples
* Ensure all safeguarding concerns aremanaged in line with Imagos Safeguarding Policy
* Ensure achievement of performancemonitoring outcomes
* Maintain excellent records using NHS andImago systems
* Undertake regular evaluation of the serviceensuring that the clients are at the heart of the process
* Maintain professional knowledge by keepingup-to-date with current legislation and changes in policy
* Undertake relevant internal and externaltraining

Person Specification
Experience
* Relevant personal or professional experience of working with children and young people and vulnerable adults in social care, health, education or voluntary and community sector
* Experience of delivering wellbeing support to clients or groups; operational experience of multi-agency working
* Proven experience of report writing, and working with individuals to identify and action personal goals; understanding and experience of monitoring and evaluation
* Ability to work independently and as part of a team; to meet objectives, achieve targets and work to tight deadlines
* Excellent communication skills; ability to stay calm in challenging situations and have a measured response
* Confident working within professional boundaries
* Ability to forge effective relationships at all levels and across boundaries
* Proficient in using Word and Excel; experience of using a database
* Good level of education
* Valid UK driving licence and access to own vehicle
* NVQ Level 3 in Health and Social Care and Information and Guidance
* Knowledge of policy and legislation relating to client group
