We are looking for a motivated staff member to work as part of a friendly, hardworking, and committed team focused on supporting and enabling patients to live independently at home following a period of acute ill health or hospitalisation.
Based at North Devon District Hospital, you will be responsible for providing a high-quality, comprehensive administrative function, including taking referrals, liaising with patients and relatives, communicating with multi-disciplinary teams, and effective data handling. You must be able to use your initiative and problem-solve, as well as work in a team.

* Provide business support, administrative leadership, and general office management to deliver a high-quality coordination service.
* Manage your workload, including financial and personnel administration, dealing with all disciplines including care providers and multi-disciplinary teams. Supervise and allocate work to staff in lower bands.
* Coordinate the Single Point of Access across North Devon to ensure delivery of a safe, high-quality service supporting individuals in crisis to prevent avoidable hospital admissions and facilitate hospital discharge pathways.
* Process Pathway 1,2,3 referrals for supported discharges within the specified response time.
* Handle Admission Avoidance referrals over the phone, gather necessary information, and signpost to appropriate services.
* Input, store, analyze information, and work within the team and with line managers to implement system changes.
* Liaise with all involved teams, wards, and agencies, including GPs, Community Teams, Hospital Discharge Teams, UCR, and Social Care.
* Ensure accurate rostering of Response and Recovery care visits, using initiative to cover staff shortages.
* Support Response and Recovery Support Workers, including lone working and outside normal hours, in line with NHS policies.
* Maintain positive relationships with colleagues and external partners.
* Interrogate IT systems (EPIC, CareFirst 6) for case information, ensure data accuracy, and communicate updates.
* Signpost less urgent referrals to appropriate teams like Social Care, Community Teams, and Nursing.
* Handle multiple information sources for two or more agencies, ensuring effective communication.
* Maintain high organizational and planning standards to meet service targets.
* Possess high-level IT skills, including EPIC, CareFirst 6, Word, Excel, and Outlook.
* Monitor and action emails, maintain accurate records, and work flexibly across the team.
Working Pattern: 8 hours per day, Monday to Friday.
