Overview
NELFT would like to introduce the launch of the Intensive Support Team, which aims to support people with a learning disability and autism who require specialist crisis intervention. The service provides care in the least restrictive environment and ensures the best response to users who need specialist support, together with their carers and families.
The post holder will work collaboratively with other LD and mental health care professionals to provide high-quality, comprehensive and dynamic specialist nursing services to maintain and improve the health and well-being of adults with learning disabilities. This includes working directly with service users with complex healthcare needs, supporting other staff, and engaging in health promotion activities. The candidate will work with multiple agencies and service providers across NELFT and will require excellent communication skills.
Responsibilities
* Member of the multi-disciplinary team and active participation in team meetings.
* Work with a wide range of health professionals and partners to reduce health inequalities and promote access to mainstream health services.
* Collaborate with individuals and their families, providing advice and support, anticipating health needs, promoting self-care and independence.
* Represent the service and service users at multi-disciplinary meetings, taking a lead role where appropriate and integrating speech and language therapy needs into a service user management plan.
* Communicate complex information effectively to service users and carers, using alternative methods of communication when appropriate.
* Provide advice and teaching to service users, their families and carers, health professionals within the team and the wider health community.
* Share information concisely to ensure safety and continuity of care.

Conditions of Service
* Probationary period applies.
* High Cost Area Supplement (Outer London) of 15% of basic salary (minimum £4,714, maximum £5,941 per annum, pro rata for part-time).
* Band 6, salary £38,682 to £46,580 per annum (plus HCAS), pro rata for part-time.
* Permanent, full-time with flexible working options.
* Location: Maggie Lilley Suite, Goodmayes Hospital, Ilford.

Qualifications and Skills
Essential
* Professional qualification - RNLD
* Evidence of postgraduate study or equivalent experience (especially in Positive Behaviour Support)
* Supervision and mentorship capabilities
* Ethical decision-making and involvement in clinical governance, audit and evidence-based practice
* Advanced report writing and IT skills to provide specialist training
* Ability to work independently and as part of a team; strong time and caseload management
Desirable
* Relevant specialist clinical skills and experience
* Experience with learning disabilities using specialist assessments and interventions
* Awareness of NHS Plan, NSF and clinical governance priorities
* Knowledge of safeguarding and NHS mandatory training
Knowledge and Abilities
Essential
* Knowledge of equality and diversity legislation
* Understanding of health inequalities and health promotion
* Training and supporting staff in MDT or multi-agency settings
* Positive valuing approach to people with learning disabilities
* Ability to manage change and support others in doing so
