Clinical Pharmacist – Hybrid Role (Up to £60,000 FTE) We are seeking an experienced and motivated Clinical Pharmacist to join a progressive and supportive primary care team within general practice. Role Overview The successful candidate will work as part of a multidisciplinary team, contributing to high-quality patient care across a range of clinical and medication-related services. The role offers a balance of on-site clinical work and remote flexibility, supporting both professional autonomy and a strong work-life balance. Key Responsibilities Conduct structured medication reviews and chronic disease medication optimisation Support long-term condition management in collaboration with GPs and the wider MDT Review prescriptions, manage repeat prescribing processes, and ensure medicines safety Provide clinical advice on complex medication queries and polypharmacy cases Participate in patient-facing consultations both in practice and remotely Support quality improvement initiatives and medicines management projects Contribute to clinical governance, audits, and service development Work collaboratively with GPs, nurses, and allied health professionals About the Role This position is based within a well-established GP practice setting, offering the opportunity to work closely with a supportive clinical team.
