We are currently looking for a dynamic and forward thinking individual to join our team as a Triage and Prevention Officer. This is an exciting opportunity to help lead and shape the implementation of our Prevention & Digital Strategic plan, which aims to build digital inclusion across adult care. The Team is responsible for the provision of Assistive Technology including careline and looking at alternative methods that could delay the need for care and support across Rochdale, enabling them to live healthier for longer.
In this exciting role, you will have the opportunity to be part of the front door team for all new contacts coming into adult care, you will focus on delaying and reducing care and support needs by supporting the front door with the delivery of assistive technology and careline.
This role is in an expanding team, which will be delivering and testing a range of existing and new initiatives whilst embedding a positive learning culture across the wider adult care services department. This role will include working with partners across Greater Manchester, health and voluntary sectors therefore excellent communication and interpersonal skills are essential.
