Barrack Lane Medical Centre has an opportunity for a care navigator to join our friendly, supportive, and professional team on a full-time basis, 37.5 hours per week. Although you will be employed by the Barrack Lane and Ivry Street PCN, you will be based at Barrack Lane.
The care navigator is a vital role within the surgery, as they are the first person a patient speaks to or sees when they contact the surgery. General practice is a challenging environment that requires a calm and caring approach, with the ability to work under pressure. The ideal candidate will be a highly motivated team player with customer service/patient experience and excellent communication skills. Previous applicants need not apply.
Main duties of the job
* Greeting and directing all patients and handing out prescriptions, letters, completed forms, and specimen bottles.
* Making appointments face-to-face, online, and via the telephone.
* Answering the telephone promptly and politely.
* Managing patient needs, dealing with inquiries, and advising on practice procedures.
* Helping patients access the appropriate services.
* Ensuring all information is recorded accurately on the computer system.
* Working as part of the care navigator team and supporting all members of the practice staff.

Barrack Lane is a busy practice located in the town centre of Ipswich, Suffolk, serving over 20,000 patients. The practice operates from two sites: the main surgery in the town centre and a branch at Stoke Park.
Barrack Lane is part of a Primary Care Network (PCN) with Ivry Street Medical Centre in Ipswich. The two practices together cover a population of 32,000 patients. The PCN allows shared resources and offers enhanced services, including care coordinators and social prescribers, to help vulnerable patients access the best care.
