$95,000.00/yr - $110,000.00/yr

Principal Duties / Essential Functions
* Monitor and complete workflow tasks in HCHB
* Code commercial OASIS
* Organize educational programs based on needs
* Review clinical records for completeness
* Collaborate with supervisory staff for documentation accuracy and effectiveness
* Lead case conferences and staff meetings
* Educate staff on audit deficiencies
* Evaluate clinical competency of nursing staff
* Participate in orientation of new team members
* Perform Blood Pressure clinics as needed
* Provide direct skilled nursing care in the field as per physician orders
Supervisory Responsibilities
* Serve as Home Care EMR resource with extended access and roles
* Supervise branch staff, conduct annual reviews, and orientations
* Train new clinicians on HCHB and homecare regulations
* Review orders and oversee care as per licensure
Qualifications
* Graduate of an accredited nursing or physical therapy school; BSN preferred, MSN strongly preferred; master's in physical therapy preferred; home care experience preferred
* Minimum 3 years’ managerial experience
* Minimum 5 years’ patient care experience
* Current RN or Physical Therapist license in Massachusetts
* CPR certification required
