An opportunity has arisen to join our Complex Discharge Support Team based in Glan Clwyd Hospital as Team Manager. The ethos is to focus on supporting patient’s home earlier from hospital where a Nursing need has been identified or a Fastrack application .

The successful candidate will lead and manage the team to ensure a seamless service is provided to ensure safe and effective discharge planning of complex patients within the Central IHC. You will have substantial experience as a complex discharge nurse and be able to coordinate and supervise the team to ensure a patient centred service.

This is an exciting and challenging role for an experience individual who is dynamic, possessing strong problem-solving skills, with the ability to deal with difficult and sensitive situations within a busy and challenging clinical setting. Be able to deliver a Discharge training programme to new starters.


Main duties of the job

The post holder must have an extensive knowledge and understanding of Home First Ethos, community services, the Discharge Process. Be an experienced Trusted assessor with excellent communication, assessment and documentation skills and be committed to working within a multidisciplinary environment.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Essential criteria

* NMC Registered Nurse
* Post Graduate Specialist Practitioner degree
* Evidence of ongoing education & professional development
* Mentorship & assessment training


Experience


Essential criteria

* Substantial experience as a discharge liasion nurse at B6 or equivalent acute & community
* Experience of working with patients who have acute/complex needs
* Varied clinical experience & multi disciplinary working


Skills


Essential criteria

* Ability to work autonomously
* Analytical skills & ability to interpret clinical signs
* Organisational skills
* Ability to assess, plan, implement, evaluate complex care needs
* Appropriate delegation of care delivery
* Supervise, teach & assess staff
* Provide mentorship & preceptorship
* Ability to work collaboratively with members of the multi disciplinary team & other agencies
* IT skills
* Excellent verbal & written communication skills
