In this role, you will play a crucial part in supporting individuals as they transition from hospital settings back into their communities. Your efforts will be vital in ensuring clients receive the appropriate support and resources to facilitate their recovery and independence.
Key Responsibilities
* Conducting assessments to identify the needs of individuals being discharged from hospital.
* Developing and implementing personalized care plans in collaboration with clients, families, and healthcare professionals.
* Coordinating with community resources and services to ensure a smooth transition and ongoing support.
* Monitoring and reviewing care plans regularly to adapt to changing needs.
* Providing emotional support and guidance to clients and their families throughout the discharge process.
Qualifications and Skills
* A recognized social work qualification (e.g., Degree in Social Work or equivalent).
* Current registration with Social Work England.
* Proven experience in adult social work, particularly within a discharge to assess context.
* Strong assessment and analytical skills, with the ability to make informed decisions.
* Excellent communication and interpersonal skills, with a compassionate approach to client care.
