Following the national release of the ARRS GP information & funding, Derbyshire Dales PCN is looking to recruit a newly qualified, (within 2 years of CCT date & not having held a substantive post before) enthusiastic & forward-thinking GP to join our Home Visiting Service on a permanent contract.
You will be required to work 5/6 sessions across the areas supported by DDPCN HVS.
DDPCN works in partnership and provides the home visiting service (Monday to Friday) for the seven GP Practices within our locality, namely:
* Evelyn Medical Centre, Hope
* Eyam Surgery
* Baslow Health Centre
* Peak and Dales Medical Partnership (Bakewell & Tideswell)
* Darley Dale Medical Centre
* Imperial Road Group Surgery
* Lime Grove Medical Centre
We are a very supportive PCN and will support you to work across the different surgeries. You will have a lead GP/Clinical Director as a mentor.
This is a great opportunity to get to know our practices across the north and central Derbyshire Dales areas and will provide the successful candidate with varied experience.
There may be opportunities to be seconded for short periods to GP Practices in our PCN Family to maximize skill development beyond the Home Visiting Service. This option is currently under review and development and can be discussed further at interview.
Main duties of the job
To perform as an autonomous general practitioner working independently and with other healthcare professionals to assess, diagnose, and treat patients who have been triaged at practice level and require an acute/same-day home visit or a proactively assessed visit outside of urgent criteria.
To provide expert professional advice to patients, carers, and colleagues, ensuring clinical excellence. To be involved in education and training of staff.
To be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct, ensuring patient safety and care quality.
The post holder must be capable of independent travel across Derbyshire Dales and work flexibly to meet business needs.

Based in Derby and Derbyshire ICB, the network covers areas including Hope, Bradwell, Eyam, Tideswell, Baslow, Bakewell, Darley Dale, Ashover, Youlgreave, and Matlock.

Job responsibilities
Clinical responsibilities
* Make yourself available to undertake consultations, telephone consultations, home visits, and administrative tasks as per the HVS rota.
* Make autonomous decisions regarding presenting problems.
* Assess healthcare needs, develop care plans, provide counselling and health education.
* Manage patient admissions/discharges and referrals.
* Record consultation notes clearly and contemporaneously.
* Compile and issue prescriptions, prescribing appropriately.
* Visit patients in emergencies if clinically indicated.
* Undertake a fair share of home visits weekly.
* Be experienced and competent with EMIS and SystmOne platforms.
Person Specification
Qualifications
* Experience in primary care, CPD, QOF, clinical audit.
* Newly qualified GP (within 2 years), not previously substantively employed in a GP practice.
* GMC registration, National Performers List registration, appropriate indemnity, and validation.
* Last appraisal evidence, eligibility to practice independently in the UK.
Skills
* Excellent communication, polite telephone manner, competent with Office/Outlook.
* Effective time management, team and autonomous working, interpersonal skills.
* Problem-solving, clinical risk management, medicines management experience.
* Full UK driver’s license.
