PCN Clinical Pharmacist – NHS, Wootton Medical Centre
Apply to become a Clinical Pharmacist for our Primary Care Network. You will work across five GP practices in Northamptonshire, providing expertise in medicines optimisation, chronic disease management and primary prescribing support.
Key Responsibilities
* Lead Long‑Term Conditions reviews and Clinical Medication Reviews in line with QOF and national guidance.
* Provide Care Home Medication Reviews and medicines safety support during hospital discharge.
* Manage repeat prescribing, acute prescription requests and medicines reconciliation.
* Support the Pharmacy and Medicines Support for patients programme, including the Prescribing Incentive Scheme.
* Collaborate with GP, community and hospital pharmacy teams to ensure integrated and efficient care.
* Deliver service development and education to practice staff, supporting evidence‑based prescribing.
* Contribute to local and national guidelines, formularies and quality improvement initiatives.
Essential Knowledge & Skills
* Excellent interpersonal, influencing and negotiation skills.
* Strong written and verbal communication, including the ability to explain complex information to patients.
* Ability to plan, monitor, advise and review medicines optimisation for long‑term conditions.
* Experience working under pressure and meeting deadlines.
* Independent prescriber or working towards competence.
* Familiarity with GP IT systems and the Medicines Safety plan.
Essential Experience
* Minimum 2‑4 years postgraduate experience in general practice or a comparable healthcare setting.
* Experience with public health promotion, antimicrobial stewardship and repeat prescribing.
* Good working knowledge of QOF, Enhanced Services and the role of GP practice in primary care.
* Experience conducting Clinical Medication Reviews and medicines reconciliation.
Qualifications
* Mandatory registration with the General Pharmaceutical Council.
* Membership of the Royal Pharmaceutical Society.
* Undergraduate Pharmacy degree and postgraduate study such as PGDip or equivalent.
* Independent Prescriber status or progressing towards it.
Additional Requirements
* Full driving licence and use of vehicle.
* Residence within reasonable distance of Wootton Medical Centre.
* UK professional registration and subject to DBS/Disclosure & Barring Service checks.
