Job summary
The Hope Specialist Falls and Respiratory Service provides apatient-centred one stop shop for people with chronic respiratory diseases,such as Chronic Bronchitis and Emphysema (COPD), and older people at risk offalls. From its inception, the service has been designed and developed througha co-production approach between a multi-disciplinary specialist team and itsusers and offers individually tailored and holistic assessment, treatment, rehabilitationand education within a community setting.

Salary Range 3£24,100 per annum. You will be appointed on a fixed salary point within the salary range.
You will provide assistance to the therapy-led Hope Specialist Servicemulti-disciplinary team (MDT) in organising and delivering efficient, effectivepatient care. This will include carrying out delegated clinical work andadministrative and clerical duties within the service. Some clinical work willbe unsupervised.
In addition, you will raiseawareness of the benefits of good health and give practical support to helppeople improve their knowledge, skills and confidence in improving theirlifestyles and managing their conditions by using community developmentprinciples and values to address and reduce health inequalities in North EastLincolnshire.
You will be educated to Literacy, Numeracy and IT level 2standard and have an NVQ Level 2 in the appropriate field e.g. Health andSocial Care (or willing to work towards this).

* Previous experience in a health care or equivalent setting which may include therapeutic support work
* Understanding of local community issues in North East Lincolnshire
* Experience of collaborative and multi-agency working
* Experience of working in a community setting
Desirable
* Experience and competency of working as an assistant / technical instructor or their equivalent.
* Understanding of medical records and therapy records keeping
