Are you passionate about making a difference in people’s lives? A rare opportunity has arisen for a Team Manager to lead our Access, Advice and Information Service, supporting individuals seeking social care support.
About the Role:
As Team Manager, you will oversee a dedicated team of social care practitioners and be the first point of contact for people looking for social care assistance. You will champion a strength-based, person-centred approach in line with the Care Act 2014, helping people achieve their desired outcomes and live as independently as possible.
Key Responsibilities:
- Manage and support the team in triaging information to provide high-quality advice and guidance to individuals and their carers.
- Develop the team’s skills to conduct initial assessments, safeguarding inquiries, and holistic assessments, including equipment provision and risk management.
- Utilise a wide range of services, including third-sector organisations, to support people’s goals.
- Ensure appropriate action is taken when individuals are at risk of harm, abuse, or neglect.
- Assist in planning sustainable contingency arrangements for emergencies and crises.
- Provide clear information about the Charging policy to help individuals and families understand financial implications of Adult Social Care services.
About You:
- Experience working in health and social care environments, ideally within advice services or the third sector.
- Proven ability to work in a fast-paced environment managing administrative systems and client interactions.
- Strong communication and organisational skills.
- Understanding of issues affecting people aged 16 and over, including older adults.
- Forward-thinking and committed to delivering high-quality, person-centred services.
