Support Practitioner - Dispersed Services
Have you had experience working as a housing support worker? Do you hold the necessary skills to empower, coach, and inspire people to move forward positively with their lives? If so, we have a fantastic opportunity for a Support Practitioner to join our dispersed team based at our head office in Liverpool.
You will be supporting vulnerable residents through a trauma-aware, strengths-based program of empowerment and confidence-building; enabling them to grow, develop, and achieve their personal goals within a safe and comfortable environment, whilst working towards greater independence and a more permanent housing solution.
Key Responsibilities of a Support Practitioner - Dispersed Services
1. Provide advice and assistance to individuals who are homeless or at risk of homelessness.
2. Conduct daytime visits, including housing-related support and tenancy sustainment within New Start’s Dispersed Service.
3. Operate within the service specification, delivery model, and contractual obligations of New Start.
4. Work within allocated time-frames for facilitating move-ins according to the applicant’s priority status.
5. Ensure all new clients receive a comprehensive induction of the property and support services, which run parallel to the tenancy.
6. Devise and implement a Service Plan with the client to map immediate support actions and lay the foundations for resettlement planning.
7. Work with clients to register with Property Pool Plus and develop a realistic resettlement plan, aiming for a 6-month outcome where appropriate.
8. Support clients in resolving debt issues.
9. Assist clients in accessing voluntary work, training, and work-based learning opportunities to help secure future employment.
10. Support clients with family mediation.
11. Create, manage, and maintain Service User files, ensuring all documentation is completed to high standards and within deadlines.
12. Initiate and carry out regular case reviews to ensure clients are on track to achieve their potential.
