Learning Disabilities CHC Social Worker
Permanent 37 hours per week £39,862–£41,771 per annum (Grade J) County Hall, Norwich
An exciting new post has been established to ensure that people with a learning disability are appropriately considered for Continuing Health Care. You will work alongside colleagues within the LD social care service to facilitate timely assessments for individuals and link with Norfolk County Councils CHC team social work team for peer support and development.
You will be a level 2 social worker with experience and understanding of the needs of people with a learning disability. You will have a level of autonomy and good communication skills.
Responsibilities
* Facilitate timely assessments for individuals with a learning disability
* Collaborate with the LD social care service and CHC team social work team for peer support and development
* Apply and maintain a level 2 social work licence and professional standards
Qualifications
* Level 2 social worker licence and experience working with people with a learning disability
* Good communication skills and ability to work autonomously
* Understanding of continuing health care service framework
