The successful candidate will join the innovative specialist nursing speciality within a highly motivated Continence Service. The team consists of nurses and an administrative secretary. They will also be working closely with the Consultant Urologists from the specialist centre from Stepping Hill providing support for patients in primary and secondary care.


Main duties of the job

The role of the Senior Continence Specialist Nurse will be to offer high level clinical assessments and reviews to patients in a nurse led community clinic and in their own home, implementing a clinical care plan to support the patient in their home environment.

The post holder will demonstrate a high level of understanding of bladder and bowel conditions and associated disorders and be able to translate that into practice.

Job responsibilities

It will be expected that the post holder will have a sound understanding of the community specialist nurse role and have a firm knowledge of utilisation of other allied health professionals within community services.

They will be expected to demonstrate that they can monitor and improve standards of care through audit and supervision, evidence based practice, learning and development.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.


Person Specification


Education & Qualifications

* Degree or Diploma in Nursing studies
* Evidence of continuing professional development including relevant attendance at postgraduate courses and conferences in nursing.
* Valid Registration with the Nursing and Midwifery Council.
* Post-graduate course in Continence, Urology or similar
* Non - Medical prescribing
* Evidence of working toward a Masters qualification or equivalent


Knowledge

* Advanced Clinical reasoning skills.
* Clinical audit.
* Understand the legal responsibilities of the profession.
* Knowledge of continence and related conditions
* Up to date in professional practice and new research.
* Awareness of NICE guidelines for Continence management.
* Demonstrate essential knowledge on how bladder and bowel medication works
* Knowledge of intermediate care and community/support services.


Experience

* Relevant experience of working with continence and associated conditions in either acute or community field.
* Previous experience in assessment of complex patients.
* Previous experience of people management.
* Experience as a mentor to staff and students on placements.
* Experience in dealing with patients in a palliative care situation.
* Have significant experience in caring for patients with bladder and bowel conditions
* Significant experience of mentorship and teaching.
* Experience of recognition of emerging cognitive impairment.
* Experience in dealing with patients in a community care situation.


Skills & Abilities

* Ability to manage complex caseload/ deal with complex issues from diagnosis to palliative care
* Knowledge of health promotion
* Ability to work autonomously and as part of an MDT
* Time management and Organisational skills.
* Skills in Clinical supervision and appraisal
* Sound understanding of clinical governance and risk.
* Ability to risk assess and ensure effective risk management strategies
* Commitment to engage in CPD
* Ability to appraise own performance
* Ability to motivate self and others.
* IT literate
* Carry out a full assessment, and if suitably qualified, prescribe medication in accordance with protocols and own competence.
* Participate in teaching staff, patients and carers about the condition(s)
* Be familiar with IT systems relevant to job role, including EMIS, Electronic health care records and dictation.
* Manage competing demands of patient, carer, and employer expectations.
* Act as a role model for others.
* Demonstrate enthusiasm, commitment and compassion in the role, using own knowledge to optimise concordance and self-care.
* Excellent interpersonal skills.
* Be able to promote examples of good practice.
* Challenge existing practice if it does not facilitate effective patient care, justifying actions.
* Have confidence in promoting new ways of thinking and working if you believe it will transform patients and carer's lives.
* Network with health professionals outside of the organisation who may help you develop your practice further.
* Work with a forward thinking approach.
* Work with other healthcare providers in Stockport to facilitate high standards of care.
* Prescribe independently in the context of a community clinic or home visit within their own sphere of professional competence and within trust guidelines.


Work Related Circumstances

* Able to work with emotional or aggressive patients and carers.
* Able to cope with frequent contact with sputum and other body fluids.
* Due to the nature of the role, it is essential that applicants are able to travel independently across Stockport and have access to a suitable vehicle for business purposes. Where possible, adjustments can be considered in accordance with the Equality Act
* Occupational Health clearance
