This is an exciting opportunity to play a key leadership role in delivering high-quality, compassionate care to patients within their own homes and communities. As a Team Lead, you will work closely with the Clinical Lead and District Nurse to ensure our service remains safe, effective, and truly person-centred.

Responsibilities
* Lead and support a dedicated team of community nursing professionals, promoting excellence in clinical practice and creating a positive, supportive team environment.
* Ensure smooth day-to-day service delivery, support staff development, and drive continuous improvement across the service.
* Provide vital support across localities to ensure that safe, high-quality care is consistently delivered within the community; oversee patient care and incidents, champion staff development, uphold professional standards, and contribute to clinical governance.
* Be highly visible in leadership, offering guidance, support, and motivation to colleagues to shape the culture and performance of the service.
* Balance clinical practice with management (approximately 40% clinical, 60% management) to stay hands-on in patient care while driving service improvement and team development.
* Collaborate with the Clinical Lead and District Nurse in governance and service delivery to ensure patient safety and positive patient and carer experience.
* Ensure patients, services users, families, and carers have a positive, safe, and high-quality experience when accessing and receiving care, treatment, advice, and support.
Requirements/Qualifications
* Band 7 Community Nursing Team Lead or equivalent experience in a community nursing leadership role.
* Ability to develop and support staff, promote professional standards, and participate in clinical governance.
* Experience of leading and managing a team in a community setting with a hands-on approach to patient care where appropriate.
* A full UK driving license and access to a vehicle for business use (reasonable adjustments available under the Equality Act 2010).
* Flexibility to work Monday to Sunday between 8am and 8pm as required.
