We are seeking a highly motivated, proactive, and dynamic individual to join our team as a Flow Facilitator at Heathlands. This is an exciting opportunity for someone with strong administrative experience who is ready to broaden their role and play a key part in supporting patient flow, discharge planning, and the day-to-day running of the unit.
The Flow Facilitator will contribute significantly to reducing patients' length of stay by enhancing communication across the multidisciplinary team (MDT), patients, and families. You will support timely, safe, and effective discharges while maintaining oversight of essential administrative processes that ensure the smooth operation of the service.
Main duties of the job
* Support patients through their admission and discharge journey, ensuring a seamless, well-coordinated experience.
* Work closely with the MDT to contribute to efficient discharge planning and promote clear, consistent communication.
* Provide guidance and support to patients' families, helping them understand the discharge process and assisting where additional support is required.
* Liaise with social services, therapy teams, and external agencies, ensuring patients receive appropriate support—whether funded or self‑funded.
* Take responsibility for day-to-day administrative tasks within the unit, helping maintain an organised and effective working environment.
* Identify delays in patient flow, elevate concerns appropriately, and help implement solutions to facilitate smoother, faster discharges.
* Uphold excellent documentation, communication, and coordination to ensure accurate records and high-quality, patient-focused service delivery.

Qualifications
* Good general education minimum to GCSE level.
Experience
* Experience as a discharge co‑ordinator or a ward clerk.
* Recent experience of working in a health or social care setting.
* Experience of working with service users with a wide range of disabilities and clinical conditions
* Experience of using EPIC.
* Experience of completing care plans.
* Experience of working with service users with life‑limiting conditions towards the end of their life.
* Experience of teaching/training.
* Experience of completing risk assessments.
Skills & Knowledge
* Experience of health and/or social care IT systems.
* Evidence of good interpersonal skills with a wide range of people in a variety of situations and settings.
* Knowledge and understanding of completing priorities.
* Experience completing patient related documentation
* Ability to demonstrate a clear understanding of differing needs of service users with different disabilities and clinical conditions.
* Experience of resolving conflict effectively.Experience of doing presentations.
* Knowledge of recent health and social care legislation and policy.
