* A Clinical Pharmacy team who undertake all prescription tasks and all medication related correspondence and Patients with Long Term Conditions

Main duties of the job
To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the GMS contract. Furthermore, the post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.

You will be joining a forward-thinking General Practice withmotivated multi-disciplinary teams of both administrative and healthcareprofessional colleagues. Horizon Healthcare operates a "Primary CareConsultant" model for our team of GPs giving value to the individual andpropelling them into a learning environment where they will have the constantsupport of an on-call partner. Administrative duties are considerably reducedby our other in-house specialist teams. Horizon Healthcare has a listsize of 28,500 plus patients and offer competitive rates of pay based onexperience and skill levels.
Job responsibilities
The followingare the core responsibilities of the salaried GP. There may be on occasion, arequirement to carry out other tasks; this will be dependent upon factors suchas workload and staffing levels:
a.The delivery of highly effective medical care to the entitled population
b.The provision of services commensurate with the GMS contract
c.Generic prescribing adhering to local and national guidance
d.Effective management of long-termconditions
e.To clinically examine and assesspatient needs from a physiological and psychological perspective and planclinical care accordingly referring patients to other services/agencies in atimely manner, utilising Practice and any other local guidelines
f.Prioritise healthproblems and intervene appropriately to assist the patient in complex, urgentor emergency situations, including initiation of effective emergency care
g.To supportpatients to adopt health promotion strategies that promote healthy lifestylesand apply principles of self-care
h.Processing of administration in a timely manner, includingreferrals, repeat prescription requests and other associated administrative tasks
i.Review medicationsfollowing the appropriate policies, NICE/local clinical guidelines and localcare pathways
j.Support thedelivery of anticipatory care plans
k.On a rotational basis, undertake telephone triage and duty doctor roles
l.Maintain accurate clinical records in conjunction with goodpractice, policy and guidance
m.Working collaboratively, accepting an equal share of the practice workload
n.Adhere to best practice recommended through clinical guidelinesand the audit process
o.Contribute to the successful implementation of continuousimprovement and quality initiatives within thepractice
p.Accept delegated responsibility for a specific area (or areas) orthe QOF
q.Attend and contribute effectively to practice meetings as required
r.Contribute effective to the development and maintenance of thepractice including; clinicalgovernance, training, financial management andHR

aa.Encourage collaborative working, liaising with all staffregularly, promoting a culture ofcontinuous improvement at all times
bb.To be an integralpart of the general practice team as well as supporting the wider network andmulti-disciplinary team
cc.There may be, onoccasion, a requirement to carry out other tasks, this will be dependent uponfactors such as workload and staffing levels
In additionto the primary responsibilities, the salaried GP may be requested to:
a.Participate in practice audits as requested by the audit lead
b.Participate in local initiatives to enhance service delivery andpatient care
c.Participate in the review of significant and near-miss eventsapplying a structured approach i.e. root cause analysis (RCA)
d.Participate in the spectrum of clinical governance processes
e.Participate in formal training events promoting best practice inarea of expertise Provide leadership and education for members of themulti-disciplinary team, providing guidance and support when necessary
Person Specification
Experience
* Experience of working in a primary care environment
* Experience of continued professional development
* Experience of QOF and clinical audit
* General understanding of the GMS contract
* Minimum of two years as a salaried GP
* Experience of medicines management
* Experience of CCG initiatives
Other requirements
* Flexibility to work outside of core office hours
* Occupational Health Clearance
* Project lead as required with CQC, CCG and QOF
Qualifications
* Qualified GP
* MRCGP
* Vocational Training Certificate or equivalent JCPTGP
* General Practitioner (Certificate of Completion of Training CCT)
* Full GMC Registration
* National Performers List registration
* Appropriate defence indemnity (MPS/MDU)
* Eligibility to practice in the UK independently
