As part of community transformation, we are changing the way we manage referrals. We have developed a referral and assessment hub and are looking to recruit a B6 Mental Health Practitioner to support management of referrals and assessments within the hub.
You will support triage completed by the senior practitioner and jointly support decision-making on the onward journey for service users. You will be aligned with one of our CMHTs but may work across all three to complete assessments.
You will provide duty assessments for the referral and assessment hub at the designated base or at a venue agreed with the service user and carer. Travel across the Salford footprint is required, including citywide travel and occasional inreach into inpatient wards.
This is a full-time position (37.5 hours per week), working from 9am to 5pm, five days a week. The role is based at Cromwell House, with travel across the borough to satellite clinics.
Key Duties Include
1. Triage of referrals into the hub, supporting onward journey decisions if not suitable for community services.
2. Completing assessments for individuals requiring specialist mental health services under the Community Mental Health Team.
3. Assessing inpatients who may need secondary services upon discharge.
4. Collaborating with the multidisciplinary team and attending weekly assessment meetings.
5. Promoting awareness of the referral and assessment hub within local services, GP practices, and the community.
6. Supporting service user, carer, and stakeholder involvement.
7. Encouraging a culture of learning, education, and professional development.
8. Championing quality assurance, including establishing standards, implementing action plans, and investigating complaints.
9. Adhering to safeguarding policies for children and vulnerable adults, and ensuring compliance among junior staff.
