Job overview

An exciting opportunity has arisen to join the Home First Team in Buckinghamshire.

The team is still in its infancy of becoming a substantive service and this provides team members with the opportunity to influence and contribute to growing and shaping the service.

We are looking for a skilled and motivated, registered health or social care professional (Physiotherapist, Occupational Therapist, Nurse or Social Worker) to join the service, which supports patients that have recently been discharged from hospital, to regain their independence and remain in their own homes.

The ideal candidate will have excellent communication skills and have experience in leading teams, developing services, quality improvement and data monitoring. The post holder will be approachable and supportive of the wider team and be able to offer advice and make decisions in the interests of our patients.

They will have knowledge and experience of discharge processes and pathways and will have the ability to accurately interpret and relay information relating to patients health and social care needs. The ability to build and maintain effective working relationships with a variety of professionals and stakeholders is essential to the role. Experience of working in both an acute and community setting would be an advantage.

The successful candidate will be required to work both remotely, on site and visit patients at home as required.

Main duties of the job

1.To provide a proactive, personalised and multidisciplinary case management approach to people who are being discharged from hospital.

2.To co-ordinate a timely, safe and effective discharge home for people which supports their ongoing care and health needs being assessed at home.

3.To work with patients and therapy staff to identify appropriate goals are set for each person and that these are reviewed, revised and updated to best support a person’s recovery using a strength-based approach.

4.To pro-actively draw on personal expertise and the skills and knowledge of all team members to improve outcomes for people and their carers.

5.To work in partnership with Home Care agencies to facilitate and support a reablement approach to care provision. Ensuring proactive right sizing of packages.

6.To proactively facilitate the person through their pathway in a way that facilitates timely discharge from home first whilst ensuring the appropriate intervention reablement/rehabilitation takes place to minimise the need for long term care.

7.To address gaps in the provision of care and changing needs through flexible and adaptable care planning, sign posting, onward referral and supported interventions.

8.To document the required information to support information sharing and monitoring of service key performance indicators.

Person specification

Education, Qualifications & Training

Essential criteria

1. Current registration with HCPC/NMC/SWE.
2. Recognised qualification in Physiotherapy/Occupational Therapy/Nursing/Social Work.
3. Clinical educator course/Practice educator course.
4. Able to evidence CPD, in line with regulatory body standards

Desirable criteria

5. Training/Formal Courses in courses appropriate to post (e.g. Continuing Healthcare)

Experience

Essential criteria

6. At least 3 – 4 years’ experience in a clinical/frontline environment (Band 6 or equivalent)
7. Experience in supervision of junior staff/students.
8. Ability to work and interact effectively within an interdisciplinary team.
9. Evidence of commitment to continuing professional development, and reflective practice

Desirable criteria

10. Experience of working in hospital discharge/discharge planning/discharge pathways

Skills, Experience & Knowledge

Essential criteria

11. Excellent communication skills (verbal & non-verbal) and interpersonal skills.
12. Ability to accurately interpret and convey information relating to patients’ health & social care needs.
13. Able to maintain accurate and contemporaneous records and documentation.
14. Effective partnership working with health and social care colleagues
15. Excellent problem solving and coordination skills.
16. Proven ability to use a variety of IT programmes and databases
17. Ability to prioritise and manage time effectively.
18. Comprehensive knowledge of discharge processes and pathways in Health & Social Care
19. Knowledge of legislation relevant to post (Care Act, Mental Capacity Act, Health and Safety, Data Protection)
20. Awareness of limitation of own professional abilities
21. Positive response to constructive criticism

25. Ability to work flexibly (weekends/bank holidays)
