What You Will Do
Provide tailored support to individuals, helping them progress along their recovery journey.
Work collaboratively with the Criminal Justice team, applying your knowledge of pathways and processes.
Carry out assessments and design personalised care plans that reflect each individual’s unique needs.
Maintain accurate records and communicate effectively to ensure continuity of care.
Engage with healthcare professionals, stakeholders, and support services to deliver joined-up care.
