Practice Nurse Chronic Disease Lead (Diabetes Focus, Prescriber Preferred)
Job Overview
Rycroft Primary Care Centre is seeking to recruit an experienced and motivated Practice Nurse to join our clinical team.
We are particularly interested in candidates with a strong background in chronic disease management, especially diabetes care, who are keen to take on a lead role in developing and delivering high-quality diabetes services within the practice.
This is an excellent opportunity for:
* An experienced Practice Nurse with independent prescribing qualification
* A nurse with a strong interest in diabetes who wishes to develop advanced clinical and leadership skills, including prescribing
The successful candidate will work as part of a well-established multidisciplinary team and play a key role in improving patient outcomes in long‑term conditions.
Main duties of the job
* Assess and manage patients’ nursing needs, including care delegated by GPs
* Deliver care in line with practice policies and national guidelines
* Work autonomously within professional competence and scope of practice
* Provide a broad range of general nursing care and refer to a GP when appropriate. Duties include: wound assessment and management, hypertension management and immunisations and vaccinations
* Support and lead chronic disease clinics including diabetes, asthma, COPD and cardiovascular disease
* Take a proactive role in developing and improving diabetes care pathways
* Monitor, review and optimise patient outcomes in line with QOF and national guidelines
* Support structured recall systems and long‑term condition monitoring
* Work collaboratively with the clinical team
* Undertake independent prescribing within scope of practice (if qualified)
* Support medication optimisation, particularly in diabetes and long‑term conditions
* Work closely with GPs and pharmacists in complex case management
* Opportunity to undertake prescribing qualification if not already held
* Cervical screening and new patient health checks
* Women’s health including HRT advice, family planning, breast awareness and swabs
* Work collaboratively with the wider primary care team to ensure high‑quality care
* Participate in clinical meetings and contribute to service development
* Support training and development of other nursing staff where appropriate

Person Specification
Experience
* Experience working within a nursing team
* Experience in chronic disease management
* Ability to work independently and as part of a multidisciplinary team
* Experience working in primary care
* Experience managing diabetes clinics or long‑term conditions
* Experience contributing to service development or quality improvement
Qualifications
* Registered Nurse with valid NMC registration
* Evidence of ongoing professional development and ability to meet revalidation requirements
* Recognised training or qualification in diabetes care
* Additional training relevant to general practice nursing is desirable
