Position: Social Worker, Community Care

About the Role
The Skye and Lochalsh Integrated Health & Social Care Team has a vacancy for a Social Worker based in its busy Portree and Broadford Offices. The team collaborates with nursing and mental health services to deliver a multidisciplinary, integrated service in a remote and rural setting.
This role offers an exciting opportunity for a qualified social worker to work with adults in a community care setting, supporting older adults, individuals with learning disabilities, autism, and those transitioning from children’s services.
Key Responsibilities
1. Supporting vulnerable adults, their families, and carers
2. Applying a person-centred approach to work with clients
3. Working within relevant legislation, including the Adult Support and Protection, Adults with Incapacity, and Mental Health Acts
4. Collaborating with NHS professionals within a multidisciplinary framework
5. Conducting assessments and report writing
6. Arranging care packages with client consent to meet assessed needs
7. Supporting adults living in a large, remote, rural area, facing potential challenges
