Hybrid working - at least 2 days in the Croydon office.(12-13 Lennard Road. CR9 2RS)
All applicants MUST have UK driving license and own vehicle for possible on site visits within the borough.
Description
-Assessing and reviewing renablement needs and goals, for people who are discharged from hospital via the Discharge to Assess (D2A) Pathway.
-Assessment of social care needs
-Assessing renaablement needs and goals
-Reviewing needs and goals
-Use Social care IT
Applicants must have:
-Knowledge of basic safeguarding legislation and processes.
-NVQ Level 2 or equivalent.
