Salaried GP - Shared Care Drug & Alcohol Service

Anexcellent opportunity has arisen for a highly motivated and committed General Practitioner (GP) to join our clinical team. This portfoliorole offers the chance to contribute tothe delivery of a high-quality shared care drug and alcohol service, supporting patients in their recovery andwellbeing journey.


Main duties of the job

Role Overview

* 4 sessions per week dedicated to drug and alcohol work
* 3 clinical sessions
* 1 administrative session

Flexible Working Options

Choose the structure that suits you:

* Option 1: 4 sessions as a Drug & Alcohol GP
* Option 2: 4 sessions as a Drug & Alcohol GP plus additional GP clinical sessions, integrated into a salaried GP role

Job responsibilities

Mainduties of the job

Work within the national clinical management guideline in line with the Drug Misuse and Dependence- UK Government on Clinical Management (Department of health & Social care 2017).

Carry out a health check at a minimum annually incorporating Hepatitis B&C, & HIV (where appropriate) and evidence of Hepatitis B vaccinations where applicable. Example include but not limited towound care, contraception, blood borne virus testing and immunisation, smokingcessation services, and mental health interventions.

Actively work with Recovery Co-ordinator to support the individual to engage in a range of interventions and treatmentoptions, to include substitute opiate prescribing and alcohol detoxificationprescribing.

To supply point of care drug screening equipment.

Ensure the person has access to and is holdingnaloxone.

To supply DBTS kits.

Hold Multi-Disciplinary Team (MDT) meeting to discuss complex cases with recovery Steps Cumbria.

Contribute to death reviews with RecoverySteps Cumbria for people within shared care.

To be part of a Peer Group Supervision supported by Recovery Steps Cumbria.

Actively engage withy the children and familyagenda, providing signposting for parenting support provision and fullyengaging with the safeguarding children agenda.

Undertake a variety of clinical sessions including face to face and telephone consultations and queries, visiting patients in their own homes, work as part of our clinical triage team and DutyDoctor.

Work with Recovery Steps Cumbria to implement community alcohol detoxification using Recovery Steps Cumbria Detoxification Pathway.

Be expected to provide general medical services to individuals who are part of shared care recovery.

Supporting people to recover

Provide information and advice on harms and risks to people using drugs and alcohol, their families and carers.

Work to reduce stigma faced by people who used drugs or alcohol.

Correctly identify people using drugs and alcohol and diagnose substance use disorders.

Carry out basic assessment of drug and alcohol use, associated strengths, harms, risk, urgency and need for referral to specialist services.

Carry out a risk assessment of suicide and harm to others and assessment of psychiatric co-morbidity and need for further medical treatment and/or onward referral.

Support people using or recovering from alcohol or drugs use in caring for their general mental and physical health and well-being.

Liaise with relevant professionals.

Provide appropriate management of assisted withdrawal where facilities allow.

Implement relevant mental health legislation for alcohol and drug users.

Provide appropriate support and interventions for families and carers of people using or receiving from alcohol and or drug use.

Prescribe appropriately for people using or recovering from alcohol or drugs, including opioid substitute prescribing and provision of medications to prevent relapse.

Clinical Governance

Be aware of research and clinical guidelines on drugs and alcohol relevant to clinical role.

Participate in research and clinical governance activities, including clinical audit.

Support service provision and development locally.

Clinical Supervision

Recovery Steps Cumbria will support share care GPs in running a multi-disciplinary meeting to discuss complex cases monthly.

Clinical supervision will be provided by Recovery Steps Cumbria via Peer Support Supervision or on a one to one if required.

GP Training Requirements

All Generalist GPs working within a shared care recovery setting will be expected to undertake initial training within the first year to include:

RCGP Certificate in the Management of Drug Misuse (Part 1) and

RCGP in the Management of Alcohol problems in Primary Care.

There is an expectation that all Generalist GPs will complete a minimum of 8 hours CPD relating to substance misuse if they remain in the role for longer than 1 year. Humankind holds an annual training event which is substance misuse focussed, this is counted in the quarterly supervision cycle but can be used toward CPD.

The CPD could include further RCGP certificates e.g. RCGP Certificate in the Management of Drug Misuse (Part 2), on-line learning and research, local substance misuse training or external learning events and conferences.

Confidentiality

In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.

In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.

Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data. Provide support for Service Delivery Leads where required.

The post holder will assist in promoting and maintaining their own and others' health, safety and security as defined in the practice Health & Safety Policy and the practice Infection Control Policy. This will include-

Using personal security systems within the workplace according to practice guidelines.

Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.

Making effective use of training to update knowledge and skills.

Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.

Actively reporting health and safety hazards and infection hazards immediately when recognised.

Keeping own workareas and general patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the jobholder's role.

Person Specification


Qualifications

* MBChB
* Completion of General Practitioner training CCT
* Full GMC Registration
* Possess the right to work in the UK and be on the performers register
* Evidence of continuous professional development
* RCGP Certificate in the Management of Drug Misuse (Part 1)
* RCGP in the Management of Alcohol problems in Primary Care


Other requirements

* Full clean driving licence


Experience

* Minimum of 5 years experience of working in primary care
* Clinical knowledge and skills commensurate with that of a GP
* Effective time management
* Ability to follow clinical guidelines, policies and procedures
* Experience with clinical risk management
* Excellent communication and interpersonal skills
* EMIS clinical system

£82,856.80 to £100,586.95 a year FTE dependant on experience
