We have an exciting opportunity for a Nursing Associate tojoin our Team. The Nursing Associate will work under thesupervision of the Practice Nursing Team.. Leadership and support will be provided by thePractice Manager, GP Partner and other members of the leadership team..
Nursing Associates are equipped with the knowledge,understanding, skills, attitudes, and behaviours to support general practice.The Nursing Associate will assist with the delivery of health promotion,phlebotomy, chronic disease management, immunisation and vaccinations and otherclinical roles in line with their training and level of experience.
Main duties of the job
Provide and monitor care,alongside the wider PCN team. contributing to the delivery of excellent patientcare, supporting the existing workforce under direct or indirect supervision.
Support registered nurses and the wider team toenable them to focus on the more complex clinical care.
To support with chronic disease clinics andmaintaining registers, deliver patient care, referring patients to othermembers to secondary/specialist care as appropriate. To implementand evaluate individual treatment plans dependant on training and experience.
Provide support and supervision to traineenursing associates, and healthcare assistants.
Accurately perform and record clinicalobservations such as blood pressure, temperature, respiration, and pulse rates.
Communicate effectively with patients regardingproviding and planning their care and carrying out risk assessment needs as required.
To act as patient advocate during practice orPCN meetings, sharing any relevant information confidently and accurately,maintaining patient confidentiality appropriately
Develop an understanding of MDT structures andmembers roles.
Support patients to improve and maintain theirphysical, mental, and behavioural health and well-being.
Maintain accurate and contemporaneous patienthealth records.

The Leadership team consists of three Partners, Practice Manager and Nursing lead and Senior mmemebrs of reception and adminitrative staff.
Job responsibilities
Work within the Practice to provide and monitor care,alongside the wider PCN team. contributing to the delivery of excellent patientcare, supporting the existing workforce under direct or indirect supervision.
Improve safety and quality of care at every opportunity.
Undertake additional clinical training asrequired by the Practice to support the Network Contract or DES Services Specification
Support registered nurses and the wider team toenable them to focus on the more complex clinical care.
To support with chronic disease clinics andmaintaining registers, deliver patient care, referring patients to othermembers of the PCN or to secondary/specialist care as appropriate. To implementand evaluate individual treatment plans dependant on training and experience.
Provide support to the Practice with regards to theirNetwork Contract DES Services Specification including CVD prevention andtackling health inequalities
Develop relationships across the Practice and PCN to supportintegration of the role across health and social care: including communitycare, secondary care, and mental health services.
Provide support and supervision to traineenursing associates, healthcare assistants, apprentices, and those on learningplacements appropriate to level of competence.
Accurately perform and record clinicalobservations such as blood pressure, temperature, respiration, and pulse rates.
Engage positively with patients/carers/familiesand staff members promoting mutually respectful and appropriate relationships.
Communicate effectively with patients regardingproviding and planning their care and carrying out risk assessment needs as required.
Communicate complex and sensitive informationverbally and in writing.
