In this rewarding role, you will be at the heart of supporting families, acting as the lead professional to coordinate and deliver early help services. You'll work closely with children, young people, parents and carers to assess needs, develop tailored support plans and ensure positive outcomes.
Key Responsibilities
* To act as the lead professional for families, children and young people requiring early help services.
* Conduct structured assessments and develop intervention plans in line with service guidance.
* Chair multi‑agency Team Around the Family/Child meetings.
* Maintain accurate and timely records, including data entry and case notes.
* Collaborate effectively with professionals across services.
* Work directly with families in a variety of settings, including their homes.
* Attend and contribute to case conferences, reviews, and planning meetings.
