Job Summary
The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies, and locality teams to deliver high standards of patient care for patients requiring support. The post holder will provide skilled nursing care under the supervision of the unplanned care advanced nurses and paramedics and undertake unplanned care community nursing duties, including holistic assessments of patients\' health needs using a person-centred approach. Work shift patterns include weekends and bank holidays; the service operates through pre-rostered shift patterns spanning 8am – 10pm, 7 days per week. The post holder will provide quality and seamless nursing care to housebound patients and coordinate their care needs to avoid unnecessary hospital admission or long-term care, while facilitating self-care for patients with long-term conditions, health promotion, and innovative activities to enhance service delivery.
Main duties of the job
* To give skilled nursing care based on best practices and clinical evidence, including catheterisation, phlebotomy, IV administration, subcutaneous injections, and palliative support.
* Act as a named nurse with supervision of the unplanned care specialist clinicians; take responsibility for holistic health needs assessment and referral to appropriate services (e.g., District planned care nurses).
* Act as the patient\'s advocate and facilitate patient choices regarding nursing care, promoting independence and self-care where appropriate.
* Identify and record new problems and report to unplanned care advanced nurses; refer as appropriate to other MDT for ongoing management.
* Maintain accurate, comprehensive, and contemporaneous records in accordance with trust and NMC guidelines.
* Provide information and support to patients, relatives and carers to promote and optimise positive health.

Skills and Knowledge
* Essential: Knowledge and evidence of catheterisation skills; risk assessment; effective communication; basic IT skills; knowledge of out-of-hospital care and unplanned care nursing; ability to listen and provide constructive feedback; general wound care; phlebotomy; IV administration; principles of care for long-term conditions.
* Desirable: Knowledge of government legislation, resource management, and audit processes
Experience
* Essential: Recent clinical nursing experience with multidisciplinary teams; working with complex health needs; general assessment; nursing process and care planning; holistic care involving MDT.
* Desirable: Experience in training others, supporting learners, involvement in auditing, research, service development; post-registration community experience
Education/Qualification
* Essential: Registered Nurse with current NMC registration
* Desirable: Evidence of additional post-registration education and training
Personal/Physical Attributes
* Essential: Full, valid UK driving licence with access to a car for business purposes (unless exempt by Equality Act 2010)
