What the Job Involves As a Care Management Social Worker, your role involves assessing the needs of vulnerable adults and their carers to arrange services that meet identified outcomes. You will promote independence, choice, and control through creative support planning and the use of personal budgets. Risk management and safeguarding are core components of your responsibilities, which include adherence to adult and children's safeguarding procedures where relevant.
