Adults Social Worker
Role: Hospital Team
Salary: £34.35 per hour
Job Type: Full time, Temporary
Hours: 9am to 5pm
About the Role:
As an Adults Social Worker, you will carry out independent assessment and support planning with the use of current legislation, theoretical models and best practice approaches.
Key responsibilities include:
* Independent assessment and Support Planning
* Work within the Transfer of Care hub for multi-disciplinary discussions and social work interventions
* Support and facilitate during hospital admission and hospital discharge
* Work in the community
* Undertake strength-based assessment and advocating support for people after discharge
Who We Are Looking For:
* Education: Degree in Social Work (or equivalent)
* Licenses: Social Work England registered
* Experience: Minimum 3 years post qualified experience in direct employment with an English Local Authority
Skills:
* Excellent communication and interpersonal skills
* Strong case management and problem-solving ability
* Resilience and empathy when working with complex cases
* Proactive team player who values safeguarding and empowerment
