In Hours Home Visiting ANP / Enhanced Practice Nurse
The Home-Visiting Advanced Nurse Practitioner/Enhanced Practice Nurse role exists to support the delivery of the Federation Home-Visiting service, working alongside Paramedic Practitioners and supported by the Service Delivery Manager, Federation HR and Admin Officers, Directors and Clinical Directors. The ANP/ACP is part of a clinical team providing home visits to patients registered with one of our sixteen practices. The ANP/ACP is a professionally autonomous practitioner managing undiagnosed problems using advanced nursing/clinical skills.
The home visiting team is an integral part of the neighbourhood working model in South West Lincolnshire. The patient cohort will predominantly be frail and complex, with co-morbidities and housebound in their own homes or living in care homes.
Responsibilities
* Deliver high-quality, autonomous care across clinical, professional and managerial domains during home visits using advanced assessment, diagnostic, and prescribing skills.
* Conduct comprehensive assessments and interpret investigations; develop care plans and promote health and well-being.
* Prioritise health needs, manage mental and community health presentations, and liaise with external agencies to support vulnerable patients.
* Safeguarding responsibilities, accurate clinical coding and documentation; maintain confidentiality and engage in continuous learning.
* Prescribe in line with current legislation and local guidelines; maintain skills in SystmOne and WebV support systems.
* Develop protocols and guidelines within the service; participate in meetings, audits, and quality improvement to support best-practice and patient-centred care.
Professional & Clinical Standards
* Practice within NMC policies; maintain patient confidentiality and continuing professional development (CPD).
* Record accurate patient contacts and ensure clinical documentation complies with legislation, policies and procedures.
* Inform on-duty GP of patient concerns during sessions; report significant events according to policy.
* Maintain accurate records of patient consultations and ensure clinical systems are up to date.
* Attend and participate in practice/PCN and multidisciplinary meetings as required.
Management & Secondary Responsibilities
* Effectively manage workload, planning, prioritising and coordinating care; work with the on-duty clinical team to maintain safe service delivery.
* Support the future development of the service; participate in local initiatives to enhance service delivery and patient care.
* Contribute to staff training, shared learning, and learning from national policies and initiatives; participate in RCA when appropriate.
Qualifications
Essential
* Registered Nurse (NMC)
* Relevant Advanced Practice Masters Degree
* Independent Prescribing Qualification
Skills
* Excellent communication, time management, teamwork and autonomous working abilities
* Interpersonal, problem-solving and analytical skills; ability to follow clinical policy
Desirable
* Strong IT skills; clear telephone manner
Personal Qualities
* Polite, confident, flexible and cooperative; high integrity and empathy; commitment to professional development
Experience
* Experience in primary care, chronic disease management, autonomous working, clinical management, service delivery, audit and research; experience with frail/complex patients
Clinical knowledge & skills
* Wound care, ECGs, venepuncture, new patient medicals, pathology testing, record-keeping, and PGD knowledge; ability to work within own scope and refer appropriately
Other Requirements
* Flexibility to work outside core hours; DBS check; complete K2 governance checks; nurse revalidation evidence; CPD; driving licence and vehicle with business use insurance
