Salary: A GBP 24,020 - GBP 30,790 per annum

They operate nearly 200 services across England with around 3,500 staff and volunteers supporting approximately 125,000 people annually.
The Role
Using a caring, compassionate, and trauma-informed approach, you will engage with individuals admitted to hospital with substance use support needs. You will deliver interventions including advice, harm reduction, and psychosocial support. You will collaborate with hospital staff to ensure appropriate support and develop discharge plans to reduce relapse and readmission.
Based within local hospitals, the role involves communication with hospital departments (e.g., Mental Health, Hepatology, Gastroenterology, A&E, Maternity) and external services (e.g., GPs, outreach, housing, community justice).
You will provide training and guidance to healthcare professionals on recognizing and managing substance-related issues, including brief interventions.
Responsibilities include:
* Attending hospital ward rounds and maintaining communication with hospital staff.
* Responding to hospital liaison referrals.
* Delivering face-to-face and phone interventions.
* Managing a caseload independently, providing psychosocial interventions.
* Participating in assessments, risk evaluations, and community referrals within a multidisciplinary team.
Skills and Qualifications
* Knowledge of substance misuse and recovery.
* Experience in workload management, planning, and delivering high-standard work.
* Understanding of safeguarding procedures.
* Excellent communication, organizational, and time-management skills.
* Ability to work effectively with diverse teams and organizations.
