The role: Actively facilitate and support the safe, effective, timely discharges for all patients within the medical specialities, improving patient flow and care experience under the direction of the band 7. To assist in the identification of appropriate boarding patients and patients who are from other catchment areas, to ensure beds are utilised effectively/ appropriately. To assist in the accurate documentation of PDDs. Support education and specialist discharge knowledge to the areas within remit. This includes all grades of nursing, medical and all other members of the multi-disciplinary team. Support the lead patient flow facilitator with service redesign and implementing change. Maintain a specialist knowledge base in relation to complex discharge issues. Support the standardisation of policy and procedure across the remit. To maintain clear, concise communication across all areas, to provide a cohesive approach to discharge planning and maintain flow throughout the hospital. Duration, Location, and Working Pattern: Permanent, Full time Royal Alexandra Hospital Shift Pattern - Monday – Friday Key responsibilities: Ensure all patients have a timely/safe discharge plan, by liaising with ward staff, all members of the multi-disciplinary team, patients, relatives, carers, primary care division and social work departments. Work closely in conjunction with bed management, supporting discharge teams, HSCPs, all specialist nurses to allow free flow of patients. This ensures appropriate utilisation of all beds. Act as a source of information for all wards and departments, closely link with Line manager /LN/CSM/GM to ensure accurate updates provided and flow maintained throughout clinical areas. Be able to work autonomously in assessing, planning, implementing and evaluating aspects of patient care. Visit all medical wards on a regular basis to proactively support all members of the multidisciplinary team with simple and complex medical discharges. Formal and informal education to all aspects of the patient flow team and potentially to all members of the multidisciplinary team. Liaise closely with all members of the multidisciplinary team to ensure a cohesive, informed assessment regarding patients’ safe discharge home. Liaise closely with colleagues from supported discharge and bed management and other services to maximise the effective utilisation of beds. Maintain and improve good working relationships and communications with colleagues in primary and secondary care and in all local authorities. Lead Medical boarders MDT when necessary. Actively participate in Proactive discharge huddles. To be accountable for own professional actions as only indirectly supervised as a specialist in patient flow. Take referrals from staff, patients and carers who require specialist advice. Ensure excellent communication and sharing of information, with the relevant personnel to make decisions regarding the discharge of medical patients. Ensures that continuous professional development meets statutory requirements and knowledge and skills framework, as per mandatory NMC regulations. Knowledge, training, qualifications and/or experience required to do the job: Registered General Nurse (1 st level) Minimum of three years post registration, with experience working in a ward environment, and a working knowledge of discharge planning. Confident IT skills, able to demonstrate ability to use various applications with ease and familiarity Excellent working knowledge of support services within the Acute Sector Excellent working knowledge of Acute Services in the Secondary Care setting Good working knowledge of Primary Care Services Evidence of continuing professional development Ability to fulfil criteria as outlined in job description.
