Brief Description:
Mental Health Clinician - Stepped Care for Older Adults
Part time 0.6 EFT fixed term 12 month maternity leave role until November 2026
Great opportunity to join a passionate multidisciplinary team and support older adults with mild to moderate mental illness in the community. Enjoy a mix of delivering individual and group interventions. Preston and other Holstep Health sites as required. Flexible hybrid work model available. $86,000 - $113,000 (pro rata) + super + salary packaging
Stepped Care for Older Adults (SCOA) program is an innovative program for adults 65 years and over (50 and over for Aboriginal and Torres Strait Islander) with mild to moderate mental illness, at risk of developing a mental illness, or experiencing social isolation and loneliness. The program operates throughout Melbourne\'s northwestern region. You will be part of a supportive multidisciplinary team who provide coordinated, integrated, and goal-oriented therapy to older adults.
You will be responsible for:
* Clinical triage including screening of referrals and biopsychosocial assessments
* Provision of evidence based time limited psychological therapies and/or interventions
* Group development and facilitation
* Care coordination (via a key worker role)
* Supporting consumers activation of choice and control
* Provide support to assist clients to connect to their immediate and wider communities
What you need to succeed:
* An approved degree in one of the following: Social Work, Clinical Psychology, Occupational Therapy or Nursing with relevant professional registration (AHPRA)
* Demonstrated clinical experience in working with older adults in a mental health setting
* Demonstrated capacity of working in a multidisciplinary team with an ability to build and maintain effective working relationships
* Knowledge of frameworks and therapies relevant to their discipline e.g. family therapy, narrative therapy and MOHO
* Excellent interpersonal skills and highly developed communication (written and verbal) skills.
