This is an exciting opportunity to join the Barnet Early Stroke Discharge Team working with a MDT including other occupational therapists, physiotherapists, psychologist, speech and language therapists and rehabilitation assistants.
The post will have a role in developing the occupational therapy practice and delivery of evidence-based Early Stroke Discharge rehabilitation packages for people returning home following initial treatment in hyper acute, acute, and tertiary stroke units serving the Barnet locality.
The ESD team works as part of Inpatients and Stroke Services pathway. The ESD team has a role to in-reach into acute units to provide advice and support to stroke patients, carers, and referring therapists.
Responsibilities include:
* Providing an early comprehensive specialist stroke service to the residents of Barnet requiring rehabilitation in the community and managing student/junior occupational therapists and other support staff.
* Being an active member of the clinical team, providing a comprehensive occupational therapy service to residents of Barnet within the Early Stroke Discharge team.
* Performing Occupational therapy assessment of patients with neurological presentations to develop and deliver an individualized treatment program.
* Working within professional standards and clinical guidelines, promoting best practice and undertaking all aspects of clinical duties as an autonomous practitioner including assessing, treating, and managing own caseload of patients/clients and maintaining professional documentation.
* Working as part of a multidisciplinary team to facilitate positive rehab outcomes, timely and coordinated discharge planning.
* Working in a variety of settings including clients’ own homes, clinics, residential and care homes, community facilities, continuing care homes and community hospitals.
* Being responsible for the supervision and line management of junior staff, rehabilitation assistants, technical instructors, and occupational therapy students.
* Supporting self and service development through active participation in continuing professional development (CPD).
