Main duties of the job
This is a rewarding, autonomous Band 6 role where your clinical judgement, case management skills and system understanding really matter. As a Commissioning Mental Health / Learning Disabilities Nurse, you will:
* Assess individuals with functional mental health needs in line with the National Framework for NHS Continuing Healthcare and Funded Nursing Care, ensuring full compliance with the Mental Health Act
* Manage and review complex packages of care, commissioning services that meet identified health outcomes
* Lead on Section 117 aftercare funding applications and oversee the commissioning of appropriate aftercare packages
* Undertake timely reviews, including regular Care Programme Approach (CPA) reviews, in line with service KPIs
* Provide robust case management for individuals eligible for CHC or other NHS‑funded care
* Work closely with acute hospitals to support Discharge to Assess pathways, ensuring safe, timely transitions of care
* Champion a consistently high standard of compassionate, person‑centred care

What We’re Looking For
* Experience in Section 117 aftercare, care commissioning and case management
* Comfortable working with complex needs and navigating multi‑agency environments
* Clear and professional communication across disciplines
* Adaptable, organised and able to prioritise in a fast‑paced setting
* Committed to partnership working and improving health outcomes
Person Specification
Experience
* Experience of working in a healthcare environment
* Previous experience of caseload management
* Track record in clinical quality and performance activities
* Experience of partnership working with other agencies
Qualifications
* Educated to degree level or equivalent level of qualification or experience in a similar specialist area
* Nursing qualification with current registration
Skills
* Understanding of Local Authority functions
* Conversant with national and local guidance on NHS continuing health care and NHS funded care
* Strong understanding of the operational management and clinical role of the NHS
* Understanding of the patient experience agenda and its contribution to quality
* Knowledge of the current NHS agenda and of primary care
* Evidence of continued professional development
* Good understanding of personalised care, in particular personal health budgets
* High level of interpersonal skills; excellent communication skills with the ability to communicate effectively with a wide range of individuals
* Capability to work across boundaries and foster close collaboration
* Ability to build effective working relationships across a range of health care and other organisations
* IT skills, ability to use Microsoft Office programmes (Word, Outlook)
* Accurate data entry and text processing skills
* Negotiation and conflict management skills and the ability to influence

* Demonstrated capability to manage own workload effectively and make informed decisions working to tight and often changing timescales
