KEY RESPONSIBILITIES:
1. Be responsible for a designated caseload, including liaising with other agencies, service users, and carers.
2. Utilize nursing knowledge and skills to assess healthcare needs of people with learning disabilities in various environments, identify healthcare needs, and implement nursing interventions.
3. Evaluate the outcomes of nursing interventions to ensure care plan objectives are met and health gains achieved.
4. Ensure all interventions are within the scope of clinical practice, with proper training and current skills guidance.
5. Provide specialist advice and training to paid and unpaid carers about conditions related to learning disabilities to improve their caring roles.
6. Undertake additional formal training to qualify as a supplementary or independent non-medical prescriber, supporting service development and client-centered care.
7. Work collaboratively with colleagues and other professionals to provide assessment, advice, and comprehensive care plans tailored to service users' needs.
8. Administer and monitor prescribed medication as part of treatment plans, observing effects and side effects.
9. Supervise and guide other nurses and professionals as necessary.
10. Support student nurses, TNAs, and other students during placements.
11. Ensure effective communication within the team.
12. Implement risk management strategies to mitigate risks effectively.
13. Contribute to external training supporting therapeutic interventions across various settings.
14. Maintain knowledge of the Mental Health Act, CTRs, LAEPs, and DSR.
15. Facilitate access to other services, including mental health admissions, and ensure appropriate involvement and decision-making.
16. Collaborate with local authorities and health providers to ensure timely discharge to community services, following protocols.
17. Advocate for service users and carers, ensuring care needs are met and concerns are escalated appropriately.
18. Facilitate access to advocacy services and IMCA where appropriate.
19. Maintain accurate records compliant with regulatory standards, Data Protection Act, and Caldicott Guidelines.
20. Complete detailed care plans, outcome measures, risk assessments, and treatment programs, involving service users, families, and multidisciplinary teams.
21. Review and evaluate care pathways and protocols regularly.
22. Act as a visible and competent role model, fostering good working relationships with partners.
23. Provide verbal and written reports for meetings such as reviews, safeguarding, and planning.
24. Participate in multidisciplinary discussions, supervision, and referral processes.
25. Support flexible working practices as needed.
