# Social Worker (Hospital Discharge Team) (Permanent) (Full-Time)Ref no:: WDN06137Published:: 15/09/2025Closes:: 29/09/2025Location:: Vale Centre for Health and Care Vale of leven General Hospital Main Street Alexandria, G83 0UASalary:: £44,710.05 - £48,396.35 per yearContract Type:: PermanentPosition Type:: Full TimeHours:: 35 hours per weekWork From Home:: Hybrid## Job DescriptionWest Dunbartonshire Health and Social Care Partnership are in a position to offer an exciting opportunity to either a newly qualified or experienced social worker within our Community Hospital Discharge Social Work Team.The RoleAs a social worker within the Community Hospital Discharge Team you will be providing a Social Work service in accordance with relevant legislation; undertaking assessments and reviews; responsibility for the care management/planning of individual packages of care; and undertaking joint working to achieve positive outcomes and meeting assessed needs to facilitate a safe discharge from hospitals across NHS Greater Glasgow and Clyde Area.All Social Workers are considered to be, and equipped as, mobile workers and can operate throughout the West Dunbartonshire area from any Council premises. You will also be required to visit people in hospital and you will work closely with professionals from health, housing, addictions and criminal justice along and in partnership with families, carers POA's and legal guardians. You will therefore be expected to travel throughout the Council area and to the Hospitals where our clients are being cared for. Social Workers will also be required to operate as a Council Officer in Adult Protection work.What We're Looking ForExcellent communication and interpersonal skills, the ability to work in a fast paced environment with the ability to implement Self Directed Support. Be able to embrace new challenges and have a flexible approach to practice. You will have experience in statutory Social Work services with a clear understanding of risk assessment. The ability to prioritise your workload and sound assessment and writing skills are also essential.
