Family & CYP Health and Wellbeing Coach - Healthy Horley PCN

This is an exciting expansion to our PCN Children and YoungPersons Wellbeing Service, and as such the successful post holder will have theopportunity to be involved in the development of the service, researchopportunities, as well as having the responsibility for collecting userfeedback and adapting the role to suit the needs of users in line with theTHRIVE model and Goal Based Outcomes (GBO) Framework.

The Health and Wellbeing Coach will be part of anetwork-based Wellbeing Team including a Social Prescribing Link Worker toengage with children, young people and families and deliver a combination ofone to one, group, and virtual interventions to improve their mental health andwellbeing.

The successful postholder will use theircoaching skills to support people to develop the knowledge, skills, andconfidence to manage their own health and wellbeing, whilst increasing theirability to access and use community support offers. Youwill work as part of a small team, undertaking initial assessments anddelivering a range of evidence-based interventions, which will include holdinga caseload.


Main duties of the job

Assess the needs and strengths of individuals referred to the service and help them identify individual goals to achieve desired change.

Coach and motivate referred patients to identify their needs, set goals, and support them to implement and achieve their personalised health and wellbeing objectives. This will include providing patients with advice, guidance and a management plan that is personalized to their individual needs, which aims to improve mental health and well-being.

Manage and prioritise a caseload of children, young people and families, in accordance with the health and wellbeing needs of the patient, including establishing and attaining goals that are important to the patient, and providing interventions to meet them.

Ensure all interventions and coaching are designed to empower patients to be active participants in their own healthcare, empower them to manage their own health and wellbeing, live independently and involve children and young people in the co-production of activities, programmes and local services.

Work with the broader MDT to maximise the support available to patients, including working collaboratively with Surrey and Borders NHS Partnership, Surrey Children's Services, Mindworks (CAMHS), Mindworks Alliance Partners, YMCA, Childrens Community Health Providers (CFHS/FCHC) and other community-based services to provide the most effective services for children, young people and their families.

Healthy Horley PCN

Our Primary CareNetwork (PCN) is an NHS collaboration between 2 GP practices East SurreyModality Medical Practice and Wayside Surgery covering Horley and surroundingareas (Outwood, Salfords and Smallfield) and has one site in Caterham. Thepractices have a combined population of 45,000, with approximately 30% ofpatients being aged between 0-25 years. There is a wide socio-economic anddemographic served within this area which will provide an interesting andvaried case mix.

The PCNdelivers a wider range of services including a Children and Young PersonsWellbeing Service, Anticipatory Care / Frailty Hub, GP Integrated Mental HealthServices, Employment Specialist Services, and a Community Health Hub (HorleyHealth Hub). We are proactively engaged with the Neighbourhood Health andWellbeing Networks.


Job responsibilities

Health andwellbeing coaches will:

Manage and prioritise a caseload ofchildren, young people and families, in accordance with the health andwellbeing needs of the patient, including establishing and attaining goals thatare important to the patient, and providing interventions to meet them.

Ensure all interventions and coaching aredesigned to empower patients to be active participants in their own healthcare,empower them to manage their own health and wellbeing, live independently andinvolve children and young people in the co-production of activities,programmes and local services.

Support and deliver group and peer sessions,with clinical and non-clinical colleagues, implementing new approaches tohealth across primary care and supporting individuals to access peer support.

Work with the broader MDT to maximise thesupport available to patients, including working collaboratively with Surreyand Borders NHS Partnership, Surrey Children's Services, Mindworks (CAMHS), MindworksAlliance Partners, YMCA, Childrens Community Health Providers (CFHS/FCHC) andother community-based services to provide the most effective services forchildren, young people and their families.

Maintain accurate records of patientengagement, evidence of change and celebrate progress with children and youngpeople.

Manageand prioritise your own caseload, in accordance with the needs, priorities andsupport required by individuals, some with complex and multiple needs.

Have agood understanding of and utilise the THRIVE delivery model in sessions,which conceptualises the mental health and wellbeing needs of children, youngpeople and families in terms of five different needs-based groupings: gettingadvice, getting help, getting more help, getting risk support, and thriving.

Applysafeguarding and child protection procedures and work closely with thesafeguarding lead to ensure any issues and concerns are escalated and dealtwith quickly and effectively.

Help provide written case studies asevidence of the effectiveness of individual interventions.

Work as part of a team and attend teammeetings, training events and participate fully in one-to-one, peer and groupsupervision.

Work across the practices within theprimary care network, including a combination of in person, remote, telephoneand video consultation.

PatientCoaching, Care and Support:

Provide personalised coaching to patientswhich supports them to identify and meet their individual lifestyle, health andwellbeing goals.

Tailor health and lifestyle adviceeffectively to the individual.

Provide personalised support toindividuals, their families and carers, and work with them holistically inmanaging challenging behaviours andresolving conflict.

Build trust and respect with the person,providing non-judgemental and nondiscriminatory support, respectingdiversity and lifestyle choices. Work from a strength-based approach focusingon a persons assets.

Increase patient motivation to self-manageand adopt healthy behaviours.

Work with patients with lower activationscores to understand and increase their level of knowledge, skills andconfidence (their Activation level) to manage their own health and wellbeingand increase their ability to access and utilise community support offers.

Be a friendly, professional and engagingsource of information about health, wellbeing and prevention approaches. Support patients in shared decision-makingconversations.

PCNand MDT

Promote health and wellbeing coaching, its role in self-management,addressing health inequalities and the wider determinants of health.

Provide education and specialist expertise to fellow PCN staff, ensuringthey are made aware of health coaching and social prescribing services andsupport colleagues to improve their skills and understanding of personalisedcare, behavioural approaches, and ensuring consistency in the follow up ofpeoples goals where an MDT is involved.

Raise awareness within the PCN of shared decision making and decisionsupport tools and supporting people in shared decision-makingconversations.

As part of the PCN multi-disciplinary team (MDT), build relationshipswith staff in GP practices within the local PCN, attending relevant MDTmeetings, giving information and feedback on health coaching.

SystemResponsibilities

Develop and maintain effective relationships with the community, ensuringthat patients can easily move between services and community resources toaccess additional support.

Work in partnership with all local agencies to raise awareness of healthand wellbeing coaching, and how partnership working can reduce pressure onstatutory services, improve health access and outcomes and enable a holisticapproach to care.

Seek regular feedback about the quality of service and impact of healthcoaching on referral agencies.

Alongside other members of the PCN multi-disciplinary team, workcollaboratively with all local diverse partners to contribute towardssupporting the local VCSE (Voluntary, Community and Social Enterprise) organisationsand community groups to become sustainable and that community assets arenurtured, through sharing intelligence regarding any gaps or problemsidentified in local provision with commissioners and local authorities.

Develop collaborative relationships and work in partnership with health,social care, and community and voluntary sector providers and multidisciplinaryteams to holistically support patients wider health and wellbeing, publichealth, and contributing to the reduction of health inequalities.

Datacapture

Work sensitively with people, their families and carers to capture keyinformation, enabling tracking of the impact of health coaching on their healthand wellbeing, including the measures required within the PCN Contract, forexample, a reduction in GP appointments for children and young people mentalhealth concerns, reducing referrals into Mindworks, and reducing repeatattendance to the GP for mental health related issues.

Through the use of patient-reported outcome measures (PROMs) andpatient-reported experience measures (PREMs); encourage people, their familiesand carers to provide feedback and to share their stories about the impact ofsocial prescribing and health coaching on their lives to help healthcareproviders, commissioners and other stakeholders to make informed changes to theservice.

Support referral agencies to provide appropriate information about theperson they are referring. Provide appropriate feedback to referral agenciesabout the people they referred.

Work closely within the MDT and with GP practices within the PCN toensure that the relevant codes are captured and inputted into clinical systems,adhering to data protection legislation and data sharing agreements.

Work with your supervisor to undertake continual personal andprofessional development, taking an active part in reviewing and developing therole and responsibilities.

We will offer peer to peer support and clinical supervision.

Undertake identified coaching and training as required.

Adhere to organisational policies and procedures, includingconfidentiality, safeguarding, lone working, information governance, equality,diversity and inclusion training and health and safety.

Other

Activelyparticipate in regular clinical supervision with a GP.

Work as part of the healthcare team to seek feedback, continually improvethe service and contribute to business planning.

Contribute to the development of policies and plans relating to equality,diversity and health inequalities.

Undertake any tasks consistent with the level of the post and the scopeof the role, ensuring that work is delivered in a timely and effectivemanner.

Person Specification


Qualifications

* Education to a good standard (Level 3)
* Level 3 qualification in coaching, mentoring, youth work, information advice and guidance or similar
* Qualification relevant to working with children, young people and families gained in education, social care or health
* Training in trauma-based cognitive behavioural therapy
* Training in motivational coaching and interviewing, or equivalent experience
* Previous training and/or willing to train in psychological interventions


Skills

* Ability to actively listen, empathise with people and provide person-centred support in a non-judgemental way
* Ability to provide motivational coaching in a way that inspires trust and confidence, supporting others to reach their potential, and promotes behaviour change
* Able to engage and communicate effectively with people, one-to-one or in group, including adjusting communication and delivery styles to an individuals needs and preferences
* Able to provide a culturally sensitive service, by supporting people from all backgrounds and communities, respecting lifestyles and diversity
* Commitment to reducing health inequalities and proactively working to reach people from diverse communities
* Ability to communicate effectively with people, their families, carers, community groups, partner agencies and stakeholders, using various methods (written, verbal, assistive technology etc.)
* Ability to identify risk and assess/manage risk when working with children, young people and families

Experience

* Minimum of two years experience of working with children, young people and their parents
* Minimum of 2 years experience of working within a primary care setting
* Experience of coaching and motivating individuals to achieve their individual health, care or lifestyle goals (including unpaid work)
* Experience of working directly in a community development or support context, including health and social care, learning support or public health/health improvement (including unpaid work)
* Experience of supporting people, their families and carers in a related role (including unpaid work)
* Experience of negotiating, managing conflict and challenging behaviours
* Experience of delivering programmes, development or support in a group context
* Experience of data collection and using tools to measure the impact of services
* Experience of working with the VCSE sector (in a paid or unpaid capacity), including with volunteers and small community groups


Knowledge

* Demonstrable knowledge in lifestyle or health interventions, for example exercise, nutrition
* Demonstrable commitment to professional and personal development
* Knowledge of the personalised care approach
* Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers
* Understanding of, and commitment to, equality, diversity and inclusion
