Community Mental Health Nurse - Named Worker Role

An exciting opportunity has arisen for a Band 6 Community Mental Health Nurse at the North Sheffield Community Mental Health Team. You will provide a range of mental health nursing intervention as part of a supportive and dynamic multi‑disciplinary team.
To assess, plan and evaluate care you need excellent communication skills and be able to work independently and as part of the wider team. A positive approach to meeting the needs of our service users and a motivating, empowering, and enthusiastic attitude to learning and sharing are essential.
Work will likely involve a duty system during normal working hours (09:00-17:00 Monday to Friday) on a rotational basis, supported by other clinical professionals and management.
Job responsibilities
* Act as a Named Worker for service users.
* Conduct comprehensive assessments of new referrals.
* Carry a varied caseload defined by need.
* Contribute to duty system on a rota basis.
* Contribute to multidisciplinary service development.
Clinical
* Carry out comprehensive assessments of individuals’ health and social care needs in various settings and agree the most appropriate course of action, including first presentation to service.
* Assess, evaluate and manage clinical risk.
* Formulate a plan of care based on identified need.
* Deliver evidence-based interventions as appropriate.
* Review effectiveness of care plan at regular intervals.
* Work within safeguarding processes, including acting as safeguarding investigator/manager when required.
* Collaborate with service users, carers and partner agencies.
* Maintain computer‑held care records accurately and timely.
* Undertake all work in a culturally sensitive manner.
* Act in accordance with Care Management System, Acute and Scheduled Care Pathways as appropriate.
* Develop effective working relationships with primary care and voluntary sector colleagues.
Person Specification – Knowledge and Skills
* Working knowledge and understanding of MHA/MCA.
* Knowledge and understanding of appropriate therapeutic approaches and engagement strategies.
* Knowledge and understanding of PDR and supervision processes as a supervisee.
* Demonstrable risk assessment and risk‑management skills with evidence of positive risk-taking.
* Additional skills/training in psychosocial interventions for people with SMI, CBTp and family interventions.
* Relevant health or social care professional qualification – Nursing.
Training and Qualifications
* Relevant health or social care professional qualification – Nursing or Social Work.
* Current registration with NMC, HCPC.
* Evidence of continued professional development.
* Supervision training.
Experience
* Experience of multi‑disciplinary team working.
* Extensive experience in mental health in both inpatient and community settings.
* Experience of managing/supervising others.
* Experience of working in a recovery‑focused way.
* Ability to work with a male‑only caseload.
* Experience of working with adults in acute/crisis/relapse presentation.
* Experience in a Community Mental Health Team/Assertive Outreach team.
