We are currently recruiting for an Outreach Recovery Worker (Harm Reduction), based in Ashford.
About The Role
Working within an integrated healthcare framework to provide support to drug and alcohol users aged 18 and over (dependent on service). Services are designed to deliver psychosocial drug and alcohol interventions for service users identified as having an alcohol or drug problem.
Services will deliver client-centred treatment using both harm reduction and abstinence-based approaches depending on the individual’s needs. You will manage a caseload, provide harm reduction advice and guidance, assess, care plan; and deliver structured 1:1 and group sessions, including structured treatment programmes. Collaboration with other services such as housing, employment, healthcare, probation, and CRCs is essential.
You will contribute to targets set by Forward and local commissioners, with outcome measurement being a key focus. Building cooperative relationships with partner agencies is vital.
Flexibility is required, including occasional evening/weekend shifts and travel to other projects in your cluster to ensure continuity of care.
Key Responsibilities
1. Provide intensive support to service users with multiple needs, including co-occurring conditions.
2. Focus primarily on the OST population at risk of disengagement, with some support for individuals from the alcohol cohort.
3. Offer additional support to prevent high-risk clients from dropping out of treatment.
4. Re-engage clients who have dropped out of treatment promptly.
5. Use an assertive outreach model to keep clients engaged in treatment.
6. Support clients in accessing and engaging with treatment, coordinating with healthcare providers to monitor health and minimize risks, including COVID-19.
7. Develop and contribute to assessment and care planning, supporting engagement with specialist agencies to address all support needs.
8. Assist clients with their care plans, including referrals to community support services, housing, benefits, and employment/training advice.
9. Promote Naloxone and needle exchange provision actively.
The Ideal Candidate Will Have
Skills and Experience
* Knowledge of OST and harm reduction.
* Experience working within substance misuse services.
* Experience conducting comprehensive assessments, risk management, and designing SMART care plans.
* Experience delivering structured interventions and facilitating groups.
* Proficiency in motivational interviewing techniques in both 1:1 and group settings.
* Strong IT skills.
* Understanding of holistic and community-based care and continuity of care.
* Experience supporting equality and diversity.
Knowledge
* Understanding issues facing substance misusers.
* Knowledge of the Recovery Agenda.
* Understanding of safeguarding practices.
Qualifications
* Level 3 Diploma in Health and Social Care.
