In this varied community-based role, you will:
* Deliver person‑centred support plans and complete dynamic risk assessments.
* Promote wellbeing, independence, and positive mental health.
* Provide practical support with daily living skills, accessing appointments, budgeting, or community engagement.
* Use therapeutic de‑escalation and crisis‑intervention techniques when needed.
* Encourage participation in meaningful activities and support individuals to work toward their personal goals.
* Maintain clear, accurate records and adhere to organisational and national policies.
* Work flexibly to meet service needs, with limited weekend commitment.
