Role Profile:
First Contact Physiotherapists (FCP) are qualified autonomous practitioners working in GP practices. They assess, diagnose, treat, and manage musculoskeletal (MSK) problems and undifferentiated conditions, and can discharge patients without medical referral when appropriate. They can be accessed directly by self-referral or through GP staff to facilitate rapid, accurate diagnosis and management, streamlining pathways of care.
Key Responsibilities:
Clinical Practice:
* Assess, diagnose, triage, and manage patients, handling complex caseloads. Progress investigations and referrals to facilitate diagnosis and treatment.
* Utilize your full scope of clinical practice, including independent prescribing, injection therapy, and investigations.
* Manage complex interactions, including patients with psychosocial and mental health needs, referring to social prescribing when needed.
* Deliver supported patient self-management programs to promote behavioral change, physical activity, mobility, independence, and reduce pharmacological interventions.
* Provide personalized care, considering each patient's overall health, well-being, employment, and functional status.
* Support patients in setting goals and building confidence in self-management.
* Communicate effectively with patients and carers about diagnosis, prognosis, and treatment options, supporting personalized care and shared decision-making.
* Accurately record data daily in the data app to monitor service utilization.
* Request patient feedback (PSQs) to monitor and share patient satisfaction metrics.
Evidence, Research, and Development:
* Build collaborative relationships across the PCN to support pathway integration in primary care.
* Work within a multidisciplinary team, using MSK expertise to enhance service links through leadership, teaching, and evaluation.
* Support research and audit initiatives to evaluate and improve the FCP service.
* Ensure timely access to care by streamlining pathways.
Leadership:
* Lead and contribute to service delivery improvements.
* Implement clinical governance practices, including audits, supervision, and training.
* Communicate complex, sensitive information effectively to patients and carers, supporting personalized care.
Facilitating Learning:
* Provide feedback and guidance to PCN team members on referral criteria.
* Reflect critically on personal practice and pursue ongoing professional development.

Experience, Knowledge, and Qualifications:
* Fully HCPC and CSP registered Physiotherapist (essential).
* At least 5 years of postgraduate clinical experience (essential).
* Minimum 3 years of MSK assessment and treatment experience (essential).
* Up-to-date CPD portfolio demonstrating knowledge of MSK core capabilities and advanced practice pillars (essential).
* Understanding of Health Education England’s Roadmap to Practice (essential).
* Level 7 MSc qualification (desirable).
* Experience in first contact triage, private sector experience considered (desirable).
* Experience working in a GP practice (desirable).
* Familiarity with EMIS / SystmOne (desirable).
* Qualified injection therapist and/or independent prescriber (desirable).
This role may include additional tasks as deemed necessary, within reasonable expectations.
