The Role We are seeking an experienced Specialist Adult Social Worker (Level 2) to join our established Neuro Rehabilitation team at Caroline House. This is a unique and rewarding opportunity to work in a highly specialist environment supporting adults recovering from acquired or traumatic brain injuries and neurological conditions at a pivotal stage of their rehabilitation journey. You will be based within Colman Hospital in Norwich although you may be required to take some community-based cases for patients with a neurological disorder. Caroline House is a well‑established neuro rehabilitation unit with a strong reputation for delivering high‑quality, person‑centred care. The service supports individuals with complex physical, cognitive and emotional needs, offering intensive rehabilitation within a warm, collaborative multidisciplinary setting. As the Specialist Social Worker within the unit, you will: Complete comprehensive, strengths‑based assessments under the Care Act 2014. Apply the Mental Capacity Act, supporting individuals with fluctuating capacity and contributing to best‑interest decisions. Lead on safe, timely and person‑centred discharge planning, working closely with hospital teams and community services. Hold a varied caseload of in‑patient rehabilitation cases and some community‑based adults with neurological conditions. Support families adjusting to sudden or life‑changing injuries, helping them understand the rehabilitation process and future options. Identify safeguarding concerns and balance risk with autonomy, recovery, and personal outcomes. Advocate for individuals’ rights, wishes, independence, and dignity. Work consistently within the principles of Community Led Support (CLS), enabling creative, community‑based solutions. We are looking for a practitioner who brings warmth, curiosity, compassion, and resilience to their practice.
