Weare looking for a motivated and enthusiastic Clinical Pharmacist to join ourfriendly, patient focused practice team.

Thesuccessful candidate will be an experienced pharmacist who, acting within theirprofessional boundaries, will provide care for the patients through medicationreconciliation, prescribing and review as appropriate. Room to develop acute orchronic management of patients is also encouraged.

Non-medicalprescribing is preferred, as well as experience working in primary care,although not essential to apply.

Main duties of the job

* Acute prescription requests,
* Prescribing and follow up
* Actioning of MHRA alerts
* Targeted medication reviews which may be done over the telephone or face to face- Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation). Make appropriate recommendations to GPs for medicine improvement.
* Answers all medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes

You will be joining a teamof 5 GP partners, 5 Salaried GPs, Nurse Practitioner, Paramedic, a ClinicalPharmacist and a team of Practice Nurses and HCAs, who are supported byManagement and the Admin Reception Team.

Job responsibilities

Patient facing or telephone- Long-term condition Clinics

See patients (whereappropriate) with single or multiplemedical problems where medicine optimisation is required (e.g. COPD,asthma).Review the on-going need for each medicine, a review of monitoringneeds and an opportunity to support patients with their medicines takingensuring they get the best use of their medicines (i.e. medicinesoptimisation).

Patient facing or telephone - Clinical Medication Review

Undertake clinical medication reviews with patients and producerecommendations for nurses and/or GP on prescribingand monitoring.

Patient facing or telephone - care home medicationreviews

Undertake clinical medication reviews with patients and producerecommendations for the nurses or GPs on prescribing and monitoring. Work withcare home staff to improve safety of medicines ordering and administration.

Patient facing domiciliary clinical medicationreview

Undertake clinical medication reviews with patients and producerecommendations for the nurses and GPs on prescribing and monitoring.

Management of common/minor/self-limiting ailments

Managing caseload of patients with common/minor/self-limiting ailments whileworking within a scope of practice and limits of competence. Signposting tocommunity pharmacy and referring to GPs or other healthcare professionals whereappropriate

Patient facing medicines support and Telephone Medicines support

Provide patient facing support and telephonehelp for patients with questions, queries and concerns about theirmedicines.

Medicine information to practice staff and patients

Answer relevant medicine-related enquiries from GPs, other practice staff,other healthcare teams (e.g. community pharmacy) and patients with queriesabout medicines. Suggesting and recommending solutions. Providing follow up forpatients to monitor the effect of any changes

Hospital admissions - planned and unplanned

Review the use ofmedicines most commonly associated with hospital admissions andreadmissions through audit and individual patient reviews. Put in place changesto reduce the prescribing of these medicines to high risk patient groups.

Management of medicines at discharge from hospital

To reconcile medicines following discharge from hospitals, intermediate careand into care homes, including identifying and rectifying unexplained changesand working with patients and community pharmacists to ensure patients receivethe medicines they need post discharge. Set up and manage systems to ensurecontinuity of medicines supply to high risk groups of patients (e.g. those withmedicine compliance aids or those in care homes).

Signposting

Ensure that patients are referred tothe appropriate healthcare professional for the appropriate level of carewithin an appropriate period of time e.g. pathology results, common/minorailments, acute conditions, long term condition reviews etc.

Repeat prescribing

Manage the repeat prescribingreauthorisation process by reviewing patient requests for repeat prescriptionsand reviewing medicines reaching review dates and flagging up those needing areview. Ensure patients have appropriate monitoring tests in place whenrequired.

Identification of cohorts ofpatients at high risk of harm from medicines through pre-prepared practicecomputer searches. This might include risks that are patient related, medicinerelated, or both.

Contribute pharmaceuticaladvice for the development and implementation of new services that havemedicinal components (e.g. advice on treatment pathways and patient informationleaflets).

Information management

Analyse, interpret andpresent medicines data to highlight issues and risks to supportdecision-making.

Undertake clinicalaudits of prescribing in areas directed by the GPs, feedback the results andimplement changes in conjunction with the practice team.

Implement changes to medicinesthat result from MHRA alerts, product withdrawal and other local and nationalguidance.

Implementation of local and national guidelines andformulary recommendations

Monitor practice prescribing against the localhealth economy's RAG list (RED, AMBER, GREEN)and make recommendations to GPs for medicines thatshould be prescribed by hospital doctors (red drugs) or subject to shared care(amber drugs).

Auditing compliance against NICE technology assessmentguidance.

Education and Training

If required, provide education andtraining to primary healthcare team on therapeutics and medicines optimisation.

Care Quality Commission

Work with practice teamsto ensure they are compliant with CQC standards where medicines are involved.

Public health - To support public healthcampaigns.

Collaborative Working Relationships

Recognises the roles of other colleagues within thePrimary Care Network and their role to patient care.

Demonstrates use of appropriate communication

Demonstrates ability to work as a member of a team.

Is able to recognise personal limitations and referto more appropriate colleague(s) when necessary.

Person Specification


Qualifications

* Completion of an Undergraduate Degree in Pharmacy and
* Registration with the General Pharmaceutical Council.
* Able to follow legal, ethical, professional and organisational
* policies/procedures and codes of conduct.
* Involves patients in decisions about prescribed medicines and
* supporting adherence as per NICE guidelines.
* Have experience and an awareness of common acute and long-term
* conditions that are likely to be seen in general practice.
* Experience working as a clinical pharmacist in General Practice.
* CPPE - Primary care pharmacy education pathway

£50,000 to £55,000 a yearDependent upon experience - pro rata
